We are currently welcoming new patients, providing Lasik, Refractive Surgery and Implant Surgery to hundreds of satisfied patients in Tampa, Clearwater, St. Petersburg, Safety Harbor, Largo, New Port Richey and surrounding areas. In an effort to facilitate your visit we ask that you fill out the requested forms and either print and bring them with you, or preferably submit online.

We understand that filling out preliminary forms can be time consuming. Prior to your visit, please take the time to complete the necessary forms. Please click on the form, and: 

1) Complete Form Online, Save to your Computer, and Submit Electronically
OR
2)  Print, Fill Out, and Bring With You to Your Appointment

Step 3: Arrive On Time with Your Insurance Card and Identification
